Application for Admission
Parents must complete admission application for School’s review in order for child to be considered for acceptance into the School’s program

Permission Agreement & Behavior Management Acknowledgement
Please complete these permission agreements, which permits the child to participate in school activities, to use the school’s play equipment, to leave the premises for supervised field trips and for school staff to take necessary steps in event of emergencies

Permission For Another Adult To Remove Child From School
Parent(s) must complete this form and provide the names of relatives or friends or care providers who are permitted to pickup and remove the child from the school

Field Trip Permission & General Field Trip Permission
Villa Divino Amore plans occasional outings and field trips under the supervision of the school staff. Please complete this form if you would like your child to participate in the field trips

Photograph Release
Please complete and sign this agreement which permits the use of your child’s school related photographs for use on our website, as well as in print material.

Emergency Medical Permission
Please complete this permission form which allows school staff to take necessary measures for the care and protection of the child in the event of emergencies

Health & Dental Assessment Records
The State of Connecticut requires these forms to be completed by a legally qualified practitioner of medicine, registered nurse or physician prior to entering into an early childhood program in Connecticut.

Dress Code
Villa Divino Amore has implemented a dress code for our students.  Please adhere to the required dress code as listed in the linked PDF.

Curriculum / Policy Handbook
Informative literature for the parent. Provides the School’s history, philosophy, goal’s and objectives; School’s dress code, medical/dental requirements and guidelines; School’s hours of operation and procedures; daily classroom schedule, routines, emergency closures, school photos, field trips and special programs.
